May be it is possible, but probably it would be not very convenient.

You'll need to have a full control over the document structure. This means

you need to disable almost all user actions except for plain text editing

and make all operations via your program user interface (with commands like

"Add tag", "Delete tag", "Move tag left/right/up/down".
