RichView.GetCheckpointItemNo returns item index of the checkpoint (I assume

that checkpoint is not in table)

RVPrint.GetFirstItemOnPage returns an index of the first item in the given

page.

You can use this information to build TOC

(important: RVPrint.GetFirstItemOnPage can be used only when document is

formatted for printing (after call of FormatPages)).
